What to bring to your first visit?

We strive to provide your child with exceptional care in a cozy and child-friendly office environment. To facilitate your initial visit, please bring your child’s insurance card, immunization record, and any relevant medical records from previous physicians. Additionally, please be prepared to fill out paperwork regarding your family medical history, your child’s medical history, and insurance information.

What to Expect

On the day of your child's first visit to our office, we recommend arriving a few minutes early. This allows extra time for finishing up any registration forms, verifying insurance, and ensuring you have plenty of time to get acquainted with our staff and office. We look forward to meeting you, answering any additional questions you may have, and building a long-term relationship with you.
